If you're a first time client please start with filling out this detailed Intake form below. When your intake form is submitted a Welcome packet will be mailed to you.

There are two options for your visit.
1. You can fill out the card in the Welcome packet and send back with your hair or nail sample in the pre-paid return envelope. Once I receive this you will be notified and will then be able to schedule your initial appointment via zoom.
2. If your local to Waupaca, WI, I will travel up to 40 miles.
You can check your preference at the end of your intake form.
