New Clients!
If you're a first time client please start with filling out this detailed Intake form below. Once this is submitted, a welcome packet will be mailed to you. You must fill out the card and send back with your hair or nail sample in the pre-paid return envelope. Once I receive this you will be notified and will then be able to schedule your appointment. Please fill out the Review of systems if you are a new client as well. This gives me a deeper look at where support is needed.
