If you're a first time client please start with filling out this detailed Intake form below. Once this is submitted, a welcome packet will be mailed to you. You must sign the Disclaimer and send back with your hair or nail sample in the pre-paid return envelope. Once I receive this sample and signed document you will be notified and will then be able to schedule your appointment.
